If you see this Tether Policy then your phone is NOT SUPPORTED (please do not waste your time trying to order)

Note: If you buy your device from Apple under Verizon contract, those devices usually have this policy. But if you buy from Verizon under verizon contract, policy is more clear. This issue is only for 6/6+ and not observed in older models yet

If your device does not activate by usual activation process, you need to do following procedure for verizon devices since they work on different frequency band.. After you receive confirmation of unlock, restore the device to latest ios version in itunes with a NON US SIM Card in the phone, the device will be unlocked and can be used outside USA thereafter.

Finally please note that after unlock your iPhone will show on GSX Check "Next Tether : Unlock" but only for iPhone 4S and before.
IF you have iPhone 5/5s/6/6+ it won't show anything, you simply need to activate the phone with a NON US SIM Card.
